spark plug blowout

2001 Protoge owner-wondering about sparkplug blowout. My daughter was driving her Protoge on a trip in -10 weather. Just had a new timing belt installed. Car had been running great. Stopped at rest stop. Came out an hour later and car was making noise. Mechanic said sparkplug had "blown out" stripping the threads. Anyone have any similar experiences?
 
Yes, thrice. Once on my wife's 2000 Protege and twice -- twice -- on my '96 Miata. Someone didn't replace the plugs correctly, i.e. stripped the threads or cross-threaded them, etc. The sad part is on the second blowout on my Miata, the plug damaged my nice aluminum hood.

On the Protege, I didn't find the plug that blew out until I did my wife's head gasket. I found it sitting in pool of its own rust deep in the engine bay.
 
Had this happen to both 1.6l motors that were in my old car. Have yet to have it happen on the 1.8l

I had this happen and used a sleeve on the plug which worked for about a year before it happened again, just this past Saturday infact. I bought the car new in January '99 and put over 160k on it. How can this be fixed, permanently? If I get a new head could it happen again?
