Spark plug blew out!

I don't know what happened!!! driving at low rpms, good oil level, everything normal. My 2001 protege 1.6 litter made a whistling sound and lost power. After pulling out on the road I openned the hood and noticed air coming out of the number two cylinder... I continued two remove the two 10mm bolts holding the sparkplug cover and what a surprise! the spark plug was shot out into the cover with melted plastic all over. I am an aricraft mechanic and I'm religious on the maintenace. I have 65000 on my car and done every sceduled maintenance (including the the 60000). Don't know what happened... did the cylinder head got o hold of the spark plug? Am I going to have to change the engine? Itsa a Mazda engine, only 65000, whats going on? please help... (boom07)
 
Loose plug is really the only way that could happen (from my experience as an aircraft mechanic ;)
 
looked in spark plug hole

I looked onto the spark plug hole and noticed that there is no threds anymore, they got stripped out when the spark plug got shot out. Do you think I'll have to change the whole engine or is there a fix for this one? I think I'll have to take it to the Mazda dealership and have a $60.00 estimate done (thiefs). Please give me some advice! The only recall I saw for the 2001 protege was a brake hose... I was hoping that was the case

you are a fricking genious!!!!!!! The dealership charged me $100.00 to give me an estimate, $1600.00 for a new head and some other stuff. So I towed it back home. I took the cylinder head off and wualaaa there was a helicoil!!!! It was a b&%$tch to take the old striped piece out but replaced with a new helicoil!! I just had to buy a new seal, the helicoil, a set of sparkplugs, and the plug with spring and sensor that goes on top of the #2 cylinder. It took me about three hours of work total and $120.00 in parts. I was about to change the whole engine or buy a new head!! thanks for your tip, it saved me lots of $$$. thanks!!

nope, I bought it new back in march 2001. This was the first problem I've ever had and all becouse I didn't put in the sparkplug tight enough.
