Keeping Good Maintenance Records

I've seem umpteen threads on here from people who have been denied warranty coverage for various reasons, among them the failure to maintain the vehicle per the maintenance schedule published in the Owner's Manual.

I bought a new Mazda3 in March and decided that I'm simply not going to let that happen. But, I do all of my own maintenance and repair, so I figured it'd be good to develop a way to keep my records in a detailed manner.

I created a maintenance incident form that I keep in a binder in my glove box. Every time I do something to the car, I fill out the form indicating what was done, and I attach any receipts for parts and/or service purchased.

I have attached the blank form in Word format along with a scan of the form I did for my first oil change. You are all welcome to use this blank form, and of course if you have feedback for me, I'd love to hear ideas for how to make it better.

Thanks :) I have always lived life by the following axiom: Whatever you can possibly imagine is the worst thing someone could possibly do to screw you over, prepare for that and you'll be fine.
 
I too keep track of everything, it was my first new car so I baby her! I use the parking brake at all times which makes everyone else angry when I let them drive my car.
My best friends dad does everything for my car he worked for BMW till he retired and he loves my 3 and takes care of her I would much rather him touch my car then anyone else.
I also make copies of EVERYTHING just in case something happens.
