Headlights go out when turn signals are turned on?????

this has happened to me 4 times since i bought the car new in 2008. i brought it to the dealer while it was still under warranty, but they found nothing. i've also noticed that my turn signals do not always sound themselves. they start to chime once i come to a stop.

just wondering if i'm not alone
