didn't see any OEM ones out there, I just picked up a replacement headlight for my '01 MPV for $22 shipped but found this.

I have a passenger headlight if that is the one you are referring to. It is brand new from Mazda, still has the plastic cover on the front and comes with a bulb and the entire rubber grommet piece.
