According to Richard at Mazdastuff.com, the lens is $110 and the whole new foglight is $105.

I have glass coverage on my car insurance. My fog light broke and I turned it into my insurance company and they replaced it. I have 100% glass coverage.
It was quite funny because at the time it was a dealer only item. The dealer ship wanted to charge to much money and the insurance Co said nope it only costs this much.
Well the dealership replaced the fog light at the price the insurance quoted them. So my replacement fog light was free to me
