Car Kills it self and jerks

My MSP has a cold air intake and a Unichip installed into. The intake is about two years old and the chip is about a year old now. About 3 months ago i started haven problems when the car is driven with a cold engine it feels like it stops giving gas to the car and looses RPMs after you shif it fixes it self for a bit then does it again till the car is warmed up. Another issue is when trying to accerlate fast around 4000 RPMs the car jerks and wont speed up then sometimes shoots forward. Any ideas?

Do you have a wideband O2 gauge?
 
How many miles are on it? Do you have the factory WGA still? Sounds like it went bad, at any point before it started to act up did you hear a whistle from the engine bay?
 
So when the WGA is broken it makes a whistling sound? It also affects the RPM and make the car jerk

This is from the Service Bulletin dealing with a ruptured WGA diaphragm:
DESCRIPTION​
Some customers may experience one or more of the following conditions:
Whistle noise from the engine compartment at idle.
Engine speed is limited by fuel cut condition under high boost conditions.
Check engine light on with P0171 - FUEL TRIM SYSTEM TOO LEAN stored in memory.​
The above mentioned conditions may be the result of a ruptured waste gate actuator diaphragm

OP might have a unichip issue or something other than a WGA issue.
