Do you mean the ones with the black housing like in my sig? If so, you can order them from Mazda Canada (quite pricey) or you can remove your front bumper and headlights and stick the headlights in the oven, then open them up and take them apart and sand/primer/paint the chrome piece like most people on here do.

You can't get those on ebay. They only have them for the sedan not the 5. MYzmzm works for a dealership in canada and she can get those for you. IMO it is cheaper and easier to do them yourself and it is not difficult and you can put that money towards something useful.
 
Yeah they have to have the separate high and low beam lights to work easily. Otherwise you have to do a mess of wiring. You said your insurance is paying for it so I guess you don't have the lights to do the painting method yourself, ouch. 100 a piece isn't a bad price at all though, good find. How much more expensive is that than the normal lights? It still might save you some to do the painting yourself, but it is quite the time consuming project if you haven't done it before and you risk not sealing the lights back up correctly which can cause issues with condensation and the like.
 
the normal headlights were 250 these are from a mazda dealer so they are factory headlights with the black insert
