grinding noise when driving

I have a premacy with 109500 miles and the gearbox sounds as if its grinding when being driven, also it is difficult to engage fourth gear and reverse when hot. Help please

1- When was the last time you changed the oil in the transmission?
2- Does the transmission seem to slip?

I would definately change and check the oil.
