Self maintenance folks - do you write a statement on what you did?

Per the manual, when do self maintenance, in order for it to count in case of warranty service, it says that you need to write a statement. Does anyone do this? Excerpt for reference :

This evidence may consist of the following: For self maintenance, a statement that you completed the maintenance yourself, displaying mileage and the date the work was performed. Also, receipts for the replacement parts (fluid, filters, etc.) indicating the date and mileage must accompany this statement."

I used to. I'd keep the receipt and would write down the mileage for my own personal records. But this really just consisted of oil changes. But now I just buy my own oil, take it to my local mechanic at lunch time and he charges me $12 for the job.
 
Yes. When my engine was replaced under warranty, I was glad I did, too.

At least snap a pic with your phone of the receipts/date.
 
Keep receipts, keep a log of what was done with date / mileage.

At least that's what I did, I never had a warranty issue so this method was never questioned. Now I'm a week away from 6 years and close to 60,000 miles. It's been a good car.
