Reporting Safety Equipment Failures to DOT/NHTSA

Here is a link for reporting abnormal operation, including failure, of any safety equipment on your vehicle.

Any time any safety equipment on your vehicle malfunctions or fails, you should file a report with the DOT. If they receive enough reports about a particular vehicle or problem, or even receive a single report of a sufficiently profound failure, they will open an investigation into whether a mandatory safety recall is necessary.

Issues that should be reported include, but are not limited to:

Brake and Brake System Failures (cracked rotors, no-response, ABS failures)
Seatbelt Failures (won't latch, won't catch, etc)
Suspension failures (broken springs, etc)
Smoking from engine compartment or interior of car (electrical burning smell, etc)
Unexplained broken glass
Unexplained airbag deployment, or failure to deploy
Unexplained tire blowouts, slipped belts, bumps, sidewall defects

Basically, any failure of any system, device, or feature on the vehicle that, if it occurred during driving, would jeopardize the safety or life of the driver or any other occupant in the vehicle, or anyone else.
