Engine threw CEL P2096...

Lastnight I threw this code P2096. After doing some research on the code it explains the car is running too lean. I cleared the code and after 50miles it came back. Now I installed my header a month ago and I already put 1500miles on it without throwing a CEL until now. So far the car feels fine when I give it gas and when it is idle. Is there any other areas I can double check? Also is there any noticable signs that will let me know that the car is running too lean?

Went underneath the car tonight and saw the bolt to my exhaust snapped off. I am not sure if this was causing the CEL. Sorry for the crappy pic.
 
Thing is I don't know if this caused the CEL. :(
 
any updates? did you fix it and the code go away? it does make sense as this would allow either air to mix with your exhaust stream or exhuast to flow out of that opening cuasing some strange flow rates would in turn cause the 02 sensor to give incorrect readings.
 
Oooh...Updates...I retighten the exhaust bolts and cleared the code. After 400miles the CEL came back.(rant) I cleared it again and the same thing happen. 400 miles...CEL. I pulled the code on my dashhawk and apparently I was giving off 2 codes instead of 1. P2096 & P0137. which is...

P2096 Post Catalyst trim Bank 1 System Too Lean
P0137 O2 Sensor Circuit Low Volts Bank 1 Sensor 2

(dunno)

I plan on checking the anti fouler and looking at the sensors again. Oh yea, I freaked out when I saw a hole in the engine block right underneath the header flange.(boom06) I checked and realized no air was coming out of it.

Since then the car has been on the dyno twice and the a/f readings looked like this...

I plan on getting a Wideband a/f ratio gauge so I'll know exactly what my a/f is.
 
those are all about your post cat 02 sensor how many antifoulers are you using 1 or 2? you may want to try different spacing. just taking a stab but i would think you are spaced to far out of the exhaust stream and thats causing your problems.
 
Are you getting bad gas mileage or your rpms falling a little during idle because my car was running lean a couple months back
 
his is running lean but its the reading in the 2nd 02 sensor not the primary so it should have no effect on the engine A/F ratio.

My gas mileage is pretty much the same as it was before. My car idles fine and my rpms aren't jumping around or bouncing all over the place.

I'm currently using 2 anti foulers. I was starting to consider using 1 or I may try trimming the current 2.

After driving back from the install meet my CEL went away. (drinks)
 
I am having the same issues, but RPMs suck and idling varies. At first I got P2096 which is a very vague code...that went away. I recently crawled under didn't see anything weird. Now car is throwing P0101 and P0137 and runnnig very poorly. Not sure what is going on but I plan to replace the downstream O2 and look into replacing the CAT which is stock on a 2006 2.0. the only mod I have is the MAF sensor rewired for my CAI. I plan to check that again...I already cleaned it a month ago.
