Help with CEL Codes

My 03.5 always have CEL codes, but I did notice 3 extra codes that are not usually there, and my vehicle is running super lean so I decided to read codes.
I did change my coils, plugs, and wires and that made everything somewhat better, no more sputtering.

P0171 System too lean Bank 1
P0455 EVAP Emission Control System Leak LARGE

How do I fix these, the first 2 seems like I may have faulty fuel injectors?

P0421
P0660
 
Your second O2 is probably failing, that or the cat is getting clogged. Evap leak is somewhat important, one of the lines for the evap system is leaking and needs to be repaired. And the P0171 is a vacuum leak that may be related to the P0455. P0421 is related to the P0140 rear O2, it's a cat efficiency code. And the P0660 is for the VICS solenoid on top of the intake manifold with the white plug.
 
Hmmm, interesting, I think I will pull up the workshop manual and see if I can find something with the EVAP Leak, and I will replace my 02 Sensor and see if that helps.
I will probably see a tech for this =/ system is running too lean $100 diagnostic fee lol
 
P0171 'system running lean' doesn't mean the car is actually running lean, it means there is a vacuum leak.
 
Hmm true... I mean the sometimes when I idle and stop at a light its like 14.9-15.3 A/F...for like 5 sec...then all of the sudden it rise sharply to 16.8-17.00 to --- A/F...Could I also reference my Boost gauge when idle it does says the normal 20 Hg
 
You definitely have a vac leak, idle should read 22-23 at least, 24 is healthy vacuum.
 
Oh dear, I need to look for a vac leak, I just went ahead and searched... I do remember first hearing a hissing noise in the engine bay, several months ago, near the intake manifold, I thought that was the Norm...
I found where the hissing came from...2 ports on the Mazda 626 Intake Manifold on the passenger front side of it. The 2 ports had existing rubber nipples that were dry rotted and is almost falling off, this needs to be taken care of. On the driver side of the car where the throttle body connects to the intake manifold there was a port that was visibly exposed that excreted the main hissing noise, I wrapped my rubber glove around the hole and looked at my A/F ratio dropped to from ~15.5 to 13.0~. Looks like its on course FROM running lean...
I am not sure if that needed to be connected to something because there is small clamp on it with the rubber ripped around the base of it. View the image below.
I might need to relocate the Vac Block, it is zip tied to the underside of the intake manifold why?!

Don't quote me on it but I think that's the port for your evap purge line. Manufacturer's usually have them located just inside the throttle body so that purged fuel vapors are evenly distributed between cylinders. Trace the vacuum line leading to your evap purge solenoid. It should have a check valve between the solenoid and wherever its hooked to an intake manifold vacuum source. If that line is not hooked up, it could be the source of your p0455 code.
