You can always replace where the MAF would do with a peice of pipe for those that have the relocated maf.

Yes you have to use the MAF, but can also do what Stealth said. And yes it does go into the driver's side fender.

It was cut off by the previous owner, pretty sure for the hi-boost FMIC kit you have to cut it. I assume it will not affect anything as long as you can get a piece of hose on it.

Honestly not sure, how do you tell? It has no book or box. Just the unit itself.

was it ever installed? check the feature lists maybe?
