Since your mazda5 is a sport, it came with halogen H7 bulbs. So, you would need the H7 HID kit from DDM. This is what I bought and am running on my 5. I am not using a relay either.

Thanks, and please let us know. This problem seems to be sporatic, so I'm sure it is hard to diagnose for the techs. Maybe get a video of the problem while it is happening with your phone if it happens.
