Unichip vacuum lines

Hey guys,
Awhile back I disconnected the vacuum lines from the 3 way valve because I thought it was causing issues with the car. Well the issue has persisted and I was wanting to hook it back up but did not take note of the configuration of the lines. I have looked all over the forums for pictures or directions for what goes where but have yet to find it. I was wondering if someone had some pictures or directions on where everything goes. I found the unichip installation instructions but it was not so helpful for knowing where the lines go. I appreciate any help!
 
Did this 3 way valve come with the unichip? Or are you talking about a vacuum 'T' that just adds a port for a tap? What kind of issue are you seeing? Also, do you have a boost gauge to verify that the vacuum is normal?
 
Here is the picture of the valve (old picture already hooked up), it did come already in the car and hooked up. The car is just a dog, I can punch the accelerator and it basically does nothing. I can hear the turbo spooling up but I can never feel it actually kick in. I am not sure what is going on with it and was going to hook it back up in hopes the chip helps the problem. Also I do not have a boost gauge to check the vacuum.

That is what controls how much boost you have. With it unhooked (no lines hooked up you will have issues building boost or controlling it) With them not hooked up you have a big vacuum leak now. It says on it (in>out>bypass) If i remember correctly or has a arrow showing direction of flow.
