97 millennia timing belt problem

Mazda millennia 2.5 v6
Hey everyone I'm new to the Mazda forums... I need some advise on what to do.. I have a 97 mil with a 2.5 v6.. Once before i had to replace the hydraulic tentioner on the timing belt. Then the other day my car stalled while driving I notice my timing belt tentioner marks are all off.. And the timing belt jumped and was being worn away. So we fixed it I start the car the timing belt jumps teeth again throwing the timing off... Does anyone have any ideas on what might be causing the belt to slip?? Oh and the belt was tight there wasn't any slack!!
 
what all did you change? i dont have any experience on the 2.5, but i have done a bunch of timing belts on the 2.3. when i do, i change all the pulleys and the tensioner. if one of the bearings in a pulley is fubar, it can make the belt jump time. looking on epc, i see 3 idlers riding the back of the belt. if the back of the belt is worn, i would check them. this is why when i do a belt, EVERYTHING gets changed.
